2. License Information

  • All driver’s licenses held in the last 3 years
  • CDL class, endorsements, restrictions
  • License numbers, states, and expiration dates

3. Driving Experience

  • Types of commercial vehicles driven
  • Years of experience
  • Approximate miles driven

4. Accident and Violation History

  • All crashes in the last 3 years
  • All moving violations in the last 3 years
  • Any suspensions or revocations

5. Employment History

  • Three years of work history for all drivers
  • Ten years of commercial driving history for CDL drivers
  • Explanations for any gaps

6. Safety Performance History Release

A signed release, usually collected with the application packet, that allows you to request prior employer safety and drug and alcohol testing information.

7. Required FMCSA Certifications

The driver must certify that all information is complete and accurate.

8. Signature and Date

Without this, the application is not valid under FMCSA rules. Even one missing required item can result in a violation.
